Eligibility Requirements
An evaluation of needs is done in the home by a qualified staff person or team.
After the needed services are identified, a case manager will help locate and arrange for the services and ensure the individual receives the needed services at the right time.
- 65 years of age or older
- Need assistance with several daily activities such as bathing, meal preparation, etc.
- Be moderately or severely disabled
- Be medically at risk of institutionalization
- Live in Frederick County
Such persons may be eligible for gap filling service dollars if they meet established financial standards and other eligibility criteria. There is no cost.
