What do I need to bring to my WIC appointment?
When you attend your appointment we need you to bring:
- All family members applying for WIC:
- Identification for all participants:
- driver's license
- birth certificate
- social security card or
- the crib card from the hospital for new babies
- Proof of your household income:
- recent family paystubs,
- medical assistance cards for all family members or
- Food Supplement Program (FSP) card - formally food stamps card
- Proof of your address:
- official mail,
- driver's license or
- copy of a lease
- Proof of pregnancy if applicable:
- doctor's note,
- pregnancy test results or
- sonogram
